El 90% restant no s\u00f3n c\u00e8l\u00b7lules humanes sin\u00f3 un aut\u00e8ntic ecosistema de, m\u00e9s o menys, 100 bilions de bacteris que habiten pac\u00edficament a l&#8217;interior de l&#8217;intest\u00ed. Reben el nom de microbioma i s\u00f3n fonamentals per a la nostra superviv\u00e8ncia. Segons el doctor Francisco Guarner responsable del grup de fisiologia i fisiopatologia digestiva de la Vall d&#8217;Hebron Institut de Recerca (VHIR) &#8220;actualment el microbioma es considera un \u00f2rgan en si mateix&#8221;.<\/p>\n<p style=\"text-align: justify;\">Alteracions en aquesta poblaci\u00f3 de microorganismes tenen serioses conseq\u00fc\u00e8ncies per a la salut. Fins a tal punt influeixen en el cos que poden arribar a modificar la conducta i el desenvolupament cerebral. Hi ha estudis que demostren que animals d&#8217;experimentaci\u00f3 que creixen en total abs\u00e8ncia de bacteris tenen un desenvolupament corporal deficient, un cervell diferent i immadur i el seu sistema immunol\u00f2gic \u00e9s incomplet. El sorprenent &#8220;i una de les raons que justifica el considerar el microbioma com a un \u00f2rgan&#8221; explica el cient\u00edfic espanyol Guarner &#8220;\u00e9s que si a aquests animals se&#8217;ls trasplanta la flora d&#8217;individus normals, recuperen la normalitat&#8221;.<\/p>\n<p style=\"text-align: justify;\"><strong>Bioqu\u00edmica cerebral<\/strong><\/p>\n<p style=\"text-align: justify;\">Aquests resultats encara no poden extrapolar-se a \u00e9ssers humans per\u00f2 &#8220;hi ha evid\u00e8ncies indirectes que el microbioma afecta la nostra bioqu\u00edmica cerebral&#8221; afirma la la investigadora i doctora Elena Verd\u00fa que ha participat en aquests experiments amb ratolins a la Universitat de McMaster, Canad\u00e0 i &#8220;\u00e9s possible que aquests mecanismes estiguin implicats en malalties com l&#8217;autisme &#8220;.<\/p>\n<p style=\"text-align: justify;\">Els trastorns gastrointestinals s\u00f3n una m\u00e9s de les complicacions que pateixen les persones amb autisme i minven la seva qualitat de vida. La causa d&#8217;aquesta associaci\u00f3 encara es desconeix per\u00f2 investigadors de la Universitat de Columbia, als EUA, han descobert el que podria ser una difer\u00e8ncia clau: molts nens autistes tenen un tipus de bacteri en la seva flora intestinal que nens sans no tenen.<\/p>\n<p style=\"text-align: justify;\">Aquest microorganisme en concret pertany al g\u00e8nere <em>Sutterella<\/em> i encara que la seva pres\u00e8ncia est\u00e0 associada a malalties digestives inflamat\u00f2ries com la malaltia de Crohn o la colitis ulcerosa segons Bren Williams, autor de l&#8217;estudi &#8220;Encara queda molta feina a fer abans d&#8217;entendre el paper de <em>Sutterella<\/em> l\u2019autisme &#8220;. El fet que una gran part de pacients autistes tinguin alterat el tipus i la quantitat d&#8217;esp\u00e8cies de la flora intestinal \u00e9s una situaci\u00f3 en la qual encara no se sap qu\u00e8 \u00e9s primer, si l&#8217;ou o la gallina. Com assegura Verd\u00fa a P\u00fablico &#8220;la connexi\u00f3 cervell-intest\u00ed \u00e9s bidireccional&#8221; i sembla prometedora.<\/p>\n<p style=\"text-align: justify;\">I \u00e9s que una flora intestinal adequada no nom\u00e9s genera vitamines i amino\u00e0cids essencials per a la superviv\u00e8ncia del cos hum\u00e0 sin\u00f3 que tamb\u00e9 estimula el sistema immunol\u00f2gic. La majoria de c\u00e8l\u00b7lules inmunocompetents conviuen amb els bacteris a la paret de l&#8217;intest\u00ed i \u00e9s principalment all\u00e0 on entren en contacte amb els ant\u00edgens de l&#8217;exterior i el sistema apr\u00e8n a diferenciar el propi de l&#8217;ali\u00e8.<\/p>\n<p style=\"text-align: justify;\">Encara que s&#8217;intueix, encara no se sap si les alteracions gastrointestinals en l&#8217;autisme s\u00f3n alguna cosa m\u00e9s que simptom\u00e0tiques per\u00f2 en altres casos s\u00ed que ho s\u00f3n. En pacients amb malaltia de Crohn o esclerosi m\u00faltiple, que s\u00ed que tenen una base autoimmune, se sap del cert que la seva flora intestinal est\u00e0 alterada i que enriquint-la milloren les seves afectacions neurol\u00f2giques. Una de les opcions terap\u00e8utiques en estudi \u00e9s restaurar la poblaci\u00f3 de bacteris i alterar el sistema immunol\u00f2gic del pacient mitjan\u00e7ant el consum de probi\u00f2tics o de determinats cucs helmints no pat\u00f2gens. Encara que aquesta \u00faltima opci\u00f3 no soni deliciosa, ja s&#8217;han obtingut resultats en diversos pacients i als EUA s&#8217;han iniciat estudis cl\u00ednics de Fase I en persones autistes i pacients amb esclerosi m\u00faltiple o amb al\u00b7l\u00e8rgies aliment\u00e0ries greus. De tota manera Guarner opina que &#8220;encara que els resultats amb els tractaments amb helmints s\u00f3n prometedors la soluci\u00f3 a aquestes malalties passa per con\u00e8ixer millor el microbioma&#8221;.<\/p>\n<p style=\"text-align: justify;\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright lazyload\" style=\"margin: 3px 8px 3px 8px;\" src=\"data:image\/gif;base64,R0lGODlhAQABAAAAACH5BAEKAAEALAAAAAABAAEAAAICTAEAOw==\" data-src=\"https:\/\/dixitciencia.com\/wp-content\/uploads\/autistmo1-1.jpg\" alt=\"\" width=\"234\" height=\"215\" \/><\/p>\n<p style=\"text-align: justify;\">Des que es coneix la repercussi\u00f3 del microbioma en el cos hum\u00e0 la investigaci\u00f3 de com alterar i obtenir efectes beneficiosos no nom\u00e9s avan\u00e7a en el camp de les malalties digestives sin\u00f3 en molts altres, com per exemple la cardiologia. Fa temps que se sap que pacients amb obesitat o diabetis tipus 2 tenen m\u00e9s risc cardiovascular que persones sanes i tamb\u00e9 elevats nivells de leptina, una hormona relacionada amb el metabolisme i la gana. Un estudi recent ha posat en pr\u00e0ctica un fet que ja es coneixia, la pres\u00e8ncia de <em>Lactobacillus plantarum<\/em> disminueix la secreci\u00f3 de leptina. Els resultats demostren que rates alimentades amb un probi\u00f2tic que cont\u00e9 aquest bacteri sintetitzen menor quantitat de leptina i aix\u00f2 podria tenir un reflex en la disminuci\u00f3 del nombre i la gravetat d&#8217;infarts. El doctor Abel Marin\u00e9, expert en nutrici\u00f3 i seguretat aliment\u00e0ria de la Universitat de Barcelona, \u200b\u200bopina que encara que &#8220;s\u00ed que sembla existir una relaci\u00f3 entre obesitat i flora intestinal i aquest estudi sigui interessant, no hem de perdre de vista que est\u00e0 fet amb animals d&#8217;experimentaci\u00f3 i que s&#8217;ha de verificar i ser reprodu\u00efble &#8220;. De tota manera &#8220;els probi\u00f2tics no s&#8217;han de confondre amb medicaments doncs els seus efectes beneficiosos s\u00f3n suaus i a llarg termini&#8221; aclareix Marin\u00e9.<\/p>\n<p style=\"text-align: justify;\"><strong>A pels gens <\/strong><\/p>\n<p style=\"text-align: justify;\">El projecte europeu MetaHIT (Metagenomics of the human intestinal tract) \u00e9s el seg\u00fcent gran pas biom\u00e8dic despr\u00e9s d&#8217;aconseguir seq\u00fcenciar el genoma hum\u00e0. El seu objectiu \u00e9s ambici\u00f3s: desxifrar el material gen\u00e8tic de les m\u00e9s de 150.000 esp\u00e8cies diferents de microbis que colonitzen el cos hum\u00e0. Guarner, responsable espanyol de MetaHIT vaticina &#8220;Anem a generar un cat\u00e0leg dels bacteris simbi\u00f2tics que viuen i participen en el nostre cos, les caracter\u00edstiques i les funcions&#8221;.<\/p>\n<p style=\"text-align: justify;\">La investigaci\u00f3 del microbioma no \u00e9s senzilla &#8220;ja que la majoria dels bacteris de l&#8217;intest\u00ed necessiten de l&#8217;ecosistema que formen amb els altres per cr\u00e9ixer&#8221; detalla Guarner a P\u00fablico &#8220;i per tant no creixen en les condicions de cultiu del laboratori&#8221;. La soluci\u00f3 a aquest repte ha estat la metagen\u00f2mica, \u00e9s a dir, aplicar t\u00e8cniques de biologia molecular i seq\u00fcenciaci\u00f3 de gens per obtenir grans bases de dades.<\/p>\n<p style=\"text-align: justify;\"><img loading=\"lazy\" decoding=\"async\" style=\"margin: 2px 8px 2px 8px;\" src=\"data:image\/gif;base64,R0lGODlhAQABAAAAACH5BAEKAAEALAAAAAABAAEAAAICTAEAOw==\" data-src=\"https:\/\/dixitciencia.com\/wp-content\/uploads\/bact-y-genes-300x225-1.jpg\" alt=\"\" width=\"240\" height=\"180\" class=\"lazyload\" \/><\/p>\n<p style=\"text-align: justify;\">Aquest projecte en qu\u00e8 participen vuit pa\u00efsos europeus es va iniciar al 2008 i t\u00e9 data prevista de finalitzaci\u00f3 per a aquest juny del 2012. Els resultats de MetaHIT fins ara s\u00f3n com a m\u00ednim sorprenents, per exemple: \u00e9s possible que els \u00e9ssers humans es puguin classificar a m\u00e9s de per el grup sanguini, el sexe i l\u2019edat, segons el seu enterotip. Els cient\u00edfics de MetaHIT van analitzar la flora intestinal de gaireb\u00e9 200 persones de sis nacionalitats diferents i van descobrir que les podrien agrupar totes en tres grups ben diferenciats. Aquest descobriment pot tenir una gran repercussi\u00f3 en medicina ja que segons explica Mani Arumugam, cient\u00edfic i primer investigador d&#8217;aquests resultats &#8220;la flora intestinal interactua directament amb els f\u00e0rmacs i els aliments que prenem i modula la seva absorci\u00f3&#8221; aix\u00ed que, especulant, Arumugam postula que &#8220;en el futur podrem dissenyar dietes i f\u00e0rmacs personalitzats en funci\u00f3 del enterotip de cada persona&#8221;.<\/p>\n<p style=\"text-align: justify;\">Els resultats de l&#8217;estudi MetaHIT pretenen tenir infinitat d&#8217;aplicacions terap\u00e8utiques &#8220;La gran perspectiva \u00e9s entendre una part del cos hum\u00e0 que fins ara desconeix\u00edem. Si arribem a con\u00e8ixer al detall aquests dos quilos de c\u00e8l\u00b7lules bacterianes que habiten en nosaltres podrem utilitzar aquest coneixement per tractar l&#8217;autisme, trastorns psiqui\u00e0trics primerencs, malalties autoimmunes, al\u00b7l\u00e8rgies, trastorns del metabolisme, obesitat, diabetis tipus 2 &#8230; &#8220;conclou Guarner.<\/p>\n<p style=\"text-align: justify;\"><strong>Els probi\u00f2tics:<\/strong><strong> \u00fatils per a unes coses i no per a unes<\/strong><strong> altres<\/strong><\/p>\n<p style=\"text-align: justify;\">Els probi\u00f2tics, terme encunyat per primera vegada el 1965, s\u00f3n microorganismes vius no pat\u00f2gens que s&#8217;han utilitzat tradicionalment en l&#8217;alimentaci\u00f3 humana. Les esp\u00e8cies de <em>Lactobacillus<\/em> i <em>Bifidobacterium<\/em> s\u00f3n les m\u00e9s usades i s&#8217;inclouen en la preparaci\u00f3 d&#8217;aliments, medicaments i suplements diet\u00e8tics.<\/p>\n<p style=\"text-align: justify;\">Aquests microorganismes no s\u00f3n habitants normals de l&#8217;intest\u00ed hum\u00e0 i encara que el seu consum no modifiqui la composici\u00f3 de la flora intestinal &#8220;en quantitats adequades milloren la digesti\u00f3 i el tr\u00e0nsit intestinal&#8221; afirma el doctor Marin\u00e9. Els seus efectes beneficiosos s&#8217;utilitzen en condicions molt concretes, com la diarrea causada per antibi\u00f2tics i en determinades malalties inflamat\u00f2ries gastrointestinals.<\/p>\n<p style=\"text-align: justify;\">En els \u00faltims anys s&#8217;ha investigat, tant amb fons p\u00fablics com privats, si els probi\u00f2tics podrien tenir efectes beneficiosos en altres camps de la salut humana. Algunes de les afirmacions m\u00e9s populars s\u00f3n inconsistents o encara han de ser recolzades per estudis ben fets en humans. Marin\u00e9 posa els punts sobres les is &#8220;el fet que un aliment tingui certs efectes beneficiosos no el converteix en un f\u00e0rmac&#8221;. El que se sap de l&#8217;efecte dels probi\u00f2tics sobre els nivells de colesterol per exemple \u00e9s encara molt contradictori.
